Death claims

Registering death claims for your loved ones is probably the hardest thing you will do. We stand by you in this difficult time and strive to ensure all our processes are simple and seamless.

Mandatory documents

Additional Mandatory documents based on manner of death

Medical/nature of death

  • Attending physician's statement

  • Medical records (admission notes, discharge/death summary, indoor case papers, test reports etc. if applicable)

For accidental or unnatural death

  • Attending physician's statement

  • Copy of the First Information Report (FIR) or Panchanama/Police complaint

  • Copy of the postmortem report (Autopsy report) & Viscera report, if performed

  • Copy of the Final Police Investigation report (FPIR) or the charge sheet

     

Hospitalization & dismemberment claim

Mandatory documents

  • Copies of hospital bills for the confinement & copies of all related clinical reports pertaining to the claim event

    • Attested true copy of Indoor Case Papers of the hospital

    • Discharge summary of present and past hospitalizations

    • Photo identity of life assured with age and address proof (List of acceptable ID and address proofs of claimant)

    • Bank details of the claimant – Cancelled cheque (with printed name and account number) / bank passbook and NEFT form

    • Medical examination certificate (First consultation notes)

    • All related clinical reports pertaining to the claim event

      • Laboratory test reports

      • X-Ray / CT Scan / MRI reports & plates

      • Ultrasonography report

      • Histopathology report

      • Clinical / hospital reports

      • Angiography reports & plates

      • Certificate of Diagnosis

    • All follow-up consultation notes in relation to the hospitalized condition

If hospitalisation is due to accidental causes (submit in addition to the above)

    • All police reports / First Information Report & Final Investigation Report
    • Proof of accident - Panchnama / Inquest report
    • Newspaper cutting / photographs of the accident, if available.

  • Disability certificate by attending physician / institute for disabled

  • Rehabilitation certificate, if applicable

  • Employer's written confirmation / statement — for disability claim

  • Clinical photographs showing the injured areas, if available

Hospitalisation is due to accidental causes (submit in addition to the above)

  • All police reports / First Information Report & Final Investigation Report — if due to accidental cause

Natural Calamity claim

  • Our heartfelt condolences to all the families who have faced inconveniences, and lost family members due to natural calamities, and recent floods in different parts of the country.

  • In this hour of grief and difficult times, Tata AIA Life Insurance stands by you, and has simplified the claim settlement process

  • For claim submission, there are only three requirements:

    1. Valid records/proof showing death of the insured member due floods, cyclones, or other natural calamities
    2. Claimant’s Statement (to be filled by nominee) with bank details of the nominee.
    3. Photo ID, Address proof of nominee and Relationship proof of the nominee with the insured

  • Proof of legal title to the claim proceeds

    (in case of no nomination (e.g. legal succession paper)

    1. FATCACRS self-certification forms—Individual

    2. FATCACRS self-certification forms—Non-Individual

    Note: We may ask for other documents, which is required on a case-by-case basis.
