    Documents required

    Proof of identity

    • Passport

    • PAN Card

    • Driving License

    • Voters ID Card

    • Aadhaar Card No. / letter issued by UIDAI.

    • Job Card issued by NREGA signed by the state Government

    In case of Natural Calamities:

    Proof of address

    • Utility Bills not older than 2 months of any service provider (electricity, telephone, post-paid mobile phone, piped gas, water bill)

    • Property/Municipal Tax Receipt

    • Bank account or Post Office savings account statement

    • Pension/Family Pension Payment Orders of retired Govt/PSU employees

    • Pension/Family Pension issued by Central/State Govt, Statutory/Regulatory Authorities, Public Sector Undertakings, Scheduled Commercial Banks, Public Financial Institutions and listed companies. Similarly, leave and license agreements with such employers allotting official accommodation.

    • Copy of the First Information Report (FIR) or Panchanama/Police complaint

    • Copy of the Copy of the Final Police Investigation report (FPIR)/Charge sheet
